On the losers front you have 1 loser in Diamonds and hopefully no more than 2 in trumps assuming trumps break kindly 3:2.

The Diamond lead followed by the friendly club switch suggests that West has a singleton Club.

Therefore, it must be vital not to let West get a Club ruff as well as the two trump tricks and the Diamond ace that has already gone

With all this in mind, at trick 2 play the club Jack which produces the Queen from East. Now play a Diamond trumping in dummy (the short hand).

Now play Heart ace followed by a Heart ruff in hand. Now play, the ♦Q, the last diamond in hand and again ruff in the short hand. Now and only now start to draw trumps, pleased to see the trumps breaking 3:2.

West plays his last top trump. You, as declarer still have one trump left (Phew) and now gets off lead with Heart King which you can trump and now club the rest with all the winning Clubs.

You made 5 clubs, 2 Diamond ruffs, 1 Heart, 1 Heart ruff and believe it or not only 1 spade.
