Initially you have 5 winners - 3 top trumps and 2 Aces.
You therefore need to find 6 more tricks
3 should come from trumps as long as trumps break no worse than 3:1. You can trump Hearts twice in dummy for two more tricks and the final trick will need to come from establishing Hearts if Hearts break 4:3 in the opponents’ hands.
With all this in mind. at trick two after winning the Ace. play a Heart.
The opponents see that you want to ruff Hearts in dummy and will switch to a trump.
However that still leaves two trumps in dummy to ruff 2 Hearts later.
You win the trump with dummy’s Queen. You now enter hand with the Diamond ace in order to ruff a Heart. After ruffing a Heart ruff a Spade to get back to hand to ruff one more Heart pleased to see Hearts now breaking.
Come back to hand with another Spade ruff and draw the last two trumps. Concede a Heart and then claim the rest.
You made 6 trumps from the log hand. 1 Diamond. 1 Spade, 2 Heart ruffs and 1 established Heart and lost only 1 top Heart and 1 further Heart trick.
(NB Had the opponents led a trump then you would be a trick short and go one down but you need to take advantage of any luck going JJ)
