Plan your play in 3NT 

Unfortunately you do not get the expected Heart lead. You will now have to rely on Clubs breaking.

You will have to keep East off lead so East cannot play a Spade through you and you will need to keep your remaining J2 intact as a possible stopper.

You cannot now take the Diamond finesse. If it fails as is likely from the bidding, East would get in and play a dastardly Spade.

Since you are going to have to finesse the Hearts and need to get to dummy you play a Diamond to dummy’s Ace and get a deserved gift when East’s King falls singleton and now you can wrap up all the Diamonds.

Despite the clubs failing to break you now make 1 Spade, 2 Hearts 5 Diamonds and 4 Clubs for an excellent score.
