If you have declarer's second suit you should lead a trump and here you need to lead a low trump.
Declarer draws trumps and then play a low Heart towards dummy and you need to play low. Declarer then plays another Heart and discovers you have all those lovely Hearts sitting over him.
As long as you are now passive and get off lead with a Diamond each time you have the lead you will eventually make 3 Heart tricks and 1 Club when partner turns up with the Club Ace.
If you were to get at all busy and not play passively declarer might make 10 tricks.
