It appears that either partner has QJ of clubs or a singleton Club.
To defeat the contract you will need to eventually make 2 clubs 1 heart and 1 Heart ruff.
With this in mind at trick two now unblock your singleton Heart Ace before playing a low club over to partner. Partner ruffs trick 3 and is on same wavelength as you and duly gives you a Heart ruff playing a low Heart (declarer not expecting such drastic distribution ruffs low but you can overruff). You can now play a second low club for partner to get a ruff. However, declarer ruffs with the Queen and draws trumps.
With no entry to dummy, declarer will eventually concede two more clubs to you with your remaining K8 sitting over his J7.
You will take the contract 3 off, winning 1 top club 1 top heart 1 Heart ruffed. 1 club ruffed and 2 further clubs at the end.
