After the §8 lead you immediately know partner has a singleton Club. (if he had started with K8 he would lead the King so declarer has the King and partner a simgleton)

Counting your tricks in defence you need 5 tricks. As long as Hearts goes round three times you will get three Heart tricks 1 Club and 1 club ruff. However if you do not take your tricks in the correct order declarer might prevail

After winning the first trick play back  High club (jack) as a suit preference for a Heart switch,

Partner ruffs the second club and duly switches to the Heart Ace. And continues Hearts. Do not try and give partner another ruff yet as declarer will merely throw a Heart loser and reduce his Heart losers to just two.

Cash the third Heart and now play a Club. Declarer trumps high, draws trumps and makes the rest.

If you do not take your tricks in the correct order declarer will only lose 3 Hearts and 1 Club or 2 Hearts and 2 clubs.
