Having listened to the bidding, it looks like West has stoppers in both Hearts and Spades for his 3NT bid. Therefore the lead looks like a singleton and your first priority is to draw trumps so as not to run into a heart ruff.
Secondly, you will need to establish dummy's Clubs and make sure you have an entry to get to them once they are established.
With this in mind, win the first trick in dummy and immediately play Ace and another Spade. East plays the King and now plays last trump.
If you were to play for the clubs to be 2:2 you would now play off two top clubs. However if you did that and the clubs were in fact 3:1 you would cut yourself off from the dummy and would inevitably lose a Club as well as a heart.
Since you can afford another loser (you are only in 5♠ despite inviting partner to 6). you should play Ace and a low club and try the Jack. Because of this excellent play you now make 12 tricks by way of 4 trumps, 2 Hearts and 6 clubs.
