After the bidding East is marked with all the outstanding points and the Club finesse should be working. But not the Diamond finesse. With this in mind we first need to test the clubs because if the clubs do break then we have 10 tricks in the bag (5 Spades, 1 Heart 2 Diamonds and 2 clubs).
So after winning the first trick with the Queen now unblock the Spades before entering dummy with the Diamond Ace. Play a third round of Spades discovering the bad news.
Now take the Club finesse which wins and cash a second Club and East follows suit with the King.
Your only hope now is to play East for an original holding of 5 cards in each minor.
At this point, render unto Caesar and exit with a Heart. East can cash three more Hearts but now has to lead away from his Q10 of Diamonds which you can let run around to dummy’s Jack. You will now make 3 Spades, 1 Heart 3 Diamonds and 2 Clubs losing just the four Hearts, for a very well played hand.
(Of course East could stop this throw in by taking the first two heart tricks and playing a third one himself).
