Initially you only have 5 winners (three Hearts and two top Diamonds)
Under these circumstances it is best to count losers and then the calculation isn’t so bad- 1 Spade loser and no other losers as you can ruff Clubs and the red suits
However you a) have a blocking problem in Hearts and b) you don’t have enough entries to get back to the dummy to enjoy the last heart BUT…
If you can ruff Diamonds in dummy, before drawing trumps, then by ruffing a Diamond, you will be able to access dummy.
Therefore, after winning the first trick with a ruff, play a Diamond to the King and a Heart back. Now ruff a Diamond (you can make the Diamond Ace later on) and return to hand with another high Heart in order to ruff another low diamond. Now it’s a mater of getting back to hand by ruffing a Heart in order to start drawing trumps.
After you have drawn trumps, you will be able to make the Diamond Ace. You will make 6 trumps, 2 Hearts, 2 Diamonds and 2 Diamonds ruffed..
If they lead trumps then have to fall back on Hearts breaking which they do
