If you are unthinking and merely lead 4th highest of your longest suit you will lead ©2 and give declarer his contract on a plate as declarer will have no choice to go up with dummy’s Queen andf hope you have led from an AK suit.
However if you lead a top Heart and have a look you will see dummy’s Doubleton Queen and partner will encourage and you will take the first four heart tricks. In order to make his contract declarer will try the Spade finesse and the contract goes down when it fails.
With a low heart lead a trick one declarer gets home with 2 Spades, 1 Heart 2 Diamonds and 4 Clubs.
The piece of advice that I would like to give on this hand is that if potentially you have an outside entry to your good suit then lead from your Ace from your AK suit. If you have no entry (most of the time) then lead 4th highest so you do not subsequently cut yourself off from partner.
