This hand comes from OK bridge and when it was played against me fortunately for me the defenders had not got their act together.
When leading from an Ace/King combination you have to agree if you want count or attitude and that is very important here.
With a non Heart lead declarer can run for home and easily make 9 tricks from Diamonds and clubs.
However if a Heart is led then you still need to be able to communicate whether or not you want the suit continued.
The received wisdom is that Ace is for Attitude (AA) and King is for Count ((KKsounds) so when you lead you really need to know how many partner has and whether it will lead to enough tricks for the defence.
You therefore lead the King for Count with declarer following suit at trick 1 with the Jack. Well the 5 looks like a count of odd cards (5 or why would declarer drop the Jack at trick one with declarer holding 2). Knowing partner has five means that declarer has just one left and he will now gets his just deserts for trying 3NT with just a doubleton QJ of Hearts and now the defence takes the first six tricks with five Hearts and one Spade|
