After winning the first trick, play the Ace and now you know partner started with a Doubleton. You must now play a “meaningful ©4” to show your entry is in the lower suit. Partner will probably work this out anyway, as you bid Diamonds, but it doesn’t hurt to emphasise the fact ,you want a Diamond with this Suit preference signal.

Partner wins with a trump and now dutifully plays a Diamond back to your King. You cash the King and then play a 4^th Heart.

With your optimum defence you will take 1 Spade, 2 Hearts, 2 Diamonds and 1 Heart  Ruffed  and 1 trump promotion to compensate for not bidding 4ª which will probably make.
