Plan your defence after declarer wins first trick with Heart Ace before going over to dummy via the Spade Ace. to finesse the Diamond which fails. Partner wins and now plays the ♣4 plan your defence.

Partner has now come back a low club promising an honour and has not played another heart so it seems partner started with a singleton Heart so he will be able to ruff your heart return.
However, the low club play tells you he has an Honour and you have the AQ and can see the Jack in dummy. so you do not wish to block the clubs.
With all this in mind now play AQ of clubs which holds. Now you can play a second Heart which partner trumps. He now underleads his Club King to make sure you ruff the 3rd Club. Why would he do that? Its because he still has another trump and wants to ruff one more Heart. He finished all this off by cashing the Spade king and you with your well thought out defence. You won 2 Clubs. 1 Diamond 1 Spade, 2 Heart ruffs and 1 club ruffed. defeating their contract by 3 tricks, and plus 300
