At trick one, if West has made a genuine 4th highest lead then you know that West must have started with KQ98 of Clubs and that your Jack or 10 is good enough to win the first trick. Alternatively you can use Rule of 11 and say 11- 8 = 3 and you can see all the cards higher than the 8 so you know East cannot have anything higher then the card led so again you can play low from dummy knowing full well the 10 is good enough to win the first trick.
After winning the first trick enter dummy with a Diamond in order to finesse the Hearts through East. When the King holds you can now play on Spades and hope that suit breaks 3:2. However the Spades do not break and East cashes another Spade before playing back a Club to partner’s lead. You win in dummy, and with hand on Heart, try another Heart finesse. East wins and cannot play another club (he has no more).
You now amass 2 Spades, 2 Hearts, 3 Diamonds and 2 Clubs for a well played, by the skin of your teeth, hand.
