After winning the first trick you must switch to trumps so that declarer cannot trump Hearts in dummy, the short hand. Declarer now seems to put all his eggs in the Diamond suit and draws the remaining trumps ending in his hand before taking the diamond finesse losing to partner's Queen who promptly plays back a Heart to your Ace. Now cash the Heart Ace before playing the Club Ace followed by another and now the contract goes 3 down.

(Perhaps declarer might have tested the Diamonds first. and it would then be up to East to switch to clubs at that point)
