5NT seemed to be highly invitational and with a very healthy maximum for his original 2♦ bid, South 6NT
After the Spade lead you have an initial count of 2 Spade winners, 5 Heart winners 1 Diamond and 3 clubs. so you will need to generate 1 more. Also you do not have entries outside the Heart suit back to your hand.
Your best chance of the extra trick needs to come from the club suit. You have 7 clubs out against you and as long as the clubs break 4:3 (62%) then you will obtain the extra trick from the 13th club.
Knowing you are going to lose a club, its now best to play low club from each hand and keep control of the actual club suit. East takes his Jack and switches to the ♦10 and you win in dummy. Now cash the clubs happy to see them break 4:3 and your last club becoming a winner.
Now engage the Hearts making sure you overtake the second Heart to be in your hand to enjoy the whole suit.
You now get home making 2 Spades, 5 Hearts, 1 Diamond 3 Clubs and 1 established club.
