This hand also comes from the Metropolitan Cup and when West, the defender, held this hand he forgot to count his winners and potential winners. After winning the first trick with the Queen which held, he continued with another Diamond. Now it was all over for the defence as declarer was able to ruff, draw trumps and concede two Hearts to West playing the Hearts through West.
Now let's rerun the hand. After winning the first trick West should know that declarer has no more Diamonds and will merely ruff the continuation and quickly draw trumps. Therefore a switch to Hearts is called for and once he switches to Hearts and partner wins and plays a second top Heart and a third one then defenders will realize 4 defensive tricks by way on 1 Diamond. 2 Hearts and 1 Heart ruffed..
