In order to make this contract you need to bring this contract home you will have to rely on the Heart finesse. If the Heart finesse fails you will be a trick short with the opponents able to amass 3 Clubs, 1 Spade and a losing Heart finesse (if its not working).

Therefore you resist any other looming finesses and put all your eggs in the Heart finessing basket. You win the second trick with dummy’s Spade Ace and take the first Heart finesse. When your Jack holds you now re-enter dummy via a top Diamond and take another Heart finesse. Some of my students do not realize that if a finesse has worked once it will work again. Having taken a second Heart finesse and seen the Hearts break 3:2 you can now claim by way of Two Spades, five Hearts and 2 Diamonds.
